Getting tested for coronavirus

If you have symptoms of coronavirus, you can book a test using the online booking system at nhs.uk/coronavirus or by calling 119. There are a number of testing centres across our area.

GP practices cannot give you a coronavirus test. If you do have symptoms, please do not come to our GP practice in person, you will be putting others at risk. Stay at home and get in touch with us over the phone or via our website.

Symptoms of coronavirus include; a new persistent cough, a high fever or loss of smell or taste.

We know that there is a high demand for tests, so please only book a test if you have symptoms.

If you have any coronavirus symptoms you must isolate immediately (stay at home) for 10 days (don’t wait for a test or a test result before doing so).

To stop the spread of coronavirus, please continue to wash your hands regularly, use a face covering when social distancing is not possible and try to keep your distance from people you do not live with.

How to book a coronavirus test

People can book a coronavirus test by going online to the NHS website - https://www.nhs.uk/conditions/coronavirus-covid-19/testing-and-tracing/get-a-test-to-check-if-you-have-coronavirus/ - or by calling 119

You must have an appointment booked before visiting a testing centre.

CHOOSE WELL

Choosing the right place to go for NHS healthcare will ensure you get the best care possible. Using an alternative to Accident and Emergency (A&E) when you aren’t seriously ill could mean that you end up waiting less time and get more efficient healthcare. It will also free up the emergency service to those who are in need of them the most. A&E and 999 are for life threatening and emergency conditions only. This can include: Broken Limbs, Heart Attacks, Strokes and other serious injuries.

MISSED APPOINTMENTS

First DNA: You will receive an initial letter to state that you have missed an appointment and the practice will be monitoring any further DNA's within a 6 month period.

Second DNA: A letter will be sent to you after you have missed a second appointment within a 6 month period of the first DNA. This letter will explain the surgery policy and that if you DNA 3 or more appointments within a 6 month period you may be deducted from the surgery list.

Third DNA: The matter will be brought to the attention of the Practice Manager and the GP's, it will then be down to their discretion if you are going to be removed or stay on the surgery list.

If you wish to cancel your appointment, please contact us on 01274 299143, at least ONE HOUR before you are due to attend. This means that we can offer the appointment to someone who needs it.

New Patient Registration

Any patient who wishes to register with the Practice should complete a Registration form (GMS1) which is available from the Reception Desk. At this time you will be given an appointment to return to the surgery with your completed registration form, together with appropriate identification ie. passport, Photographic Driving Licence and a utility bill. When you attend this appointment a copy of your documents will be taken and your completed registration form will be checked. You can register with the practice if you live within the practice area. A map showing the area is available in the waiting area.

Online 'pre-registration' with the practice

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Please note that completion of this on-line form does not mean you are automatically accepted onto the practice list.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
